What is the I-693 Exam?
Purpose of Form I-693:
Medical Examination of Aliens Seeking Adjustment of Status
Form I-693 must be filed by individuals that apply for adjustment of status.
Form I-693 can only be completed by a physician that is approved by the
United States Center for Immigration Services. All expenses related to the health check and completion of Form
I-693 must be paid by the applicant. An adjustment of status application is not
complete unless Form I-693 is included and approved by the physician. Form I-693
(Medical Examination of Aliens Seeking Adjustment of Status) may also be known
as USCIS Form I-693, I 693, Form I693 or immigration form I-693.
Dr. Sheta can help you complete your I-693 Exam. Civil surgeons are responsible
for ensuring that all of the required components of the I-693 form have been
completed. The civil surgeon will record the exam results on Form I-693 for the
applicant to return to the USCIS (INS).
Each evaluation includes an examination performed according to the medical
guidelines established by the USCIS and Centers for Disease Control; all
required laboratory testing; TB testing and interpretation; and completion of
all the necessary paperwork. If necessary, vaccinations and chest x-rays are
performed for an additional fee.
Dr. Sheta is experienced with the medical requirements of these examinations.
It is mandatory that immigration physicals are completed by a Designated Civil
Surgeon so the exam will be valid. To accommodate cultural preferences, male
and female Designated Civil Surgeons are available.
THE USCIS I-693
EXAM The procedure for changing your immigrant
status has four steps: STEP 1

Go to the office of Dr. Sheta.

You will receive a blood and skin test in accordance with
INS procedures

Bring any vaccination records you have

Visit fee is $100.00* (cash or credit card)

Get a physical from Dr. Sheta
STEP 2
-
Go to the office of Dr. Sheta again (two or three days
later).

Dr. Sheta will review the results with you.

Skin Test results: If negative = Done (no Chest x-ray needed) If positive = Chest x-ray required

Determine which vaccine(s) are needed (if any) based on INS
policy (You will be referred to the proper Medical Clinic to get your
vaccines.)
STEP 3

Once you have gotten required vaccines, bring the paperwork
to our office

Dr. Sheta will complete the paperwork, sign and seal the
envelope

Bring the sealed envelope to the USCIS Department in
Las Vegas

Pricing
The price $100.00 for the procedure. This includes a USCIS physical exam, a USCIS
health history, a PPD skin test and the completion of your USCIS-693 paperwork
handed to you in a sealed envelope. LAB FEES:
Any laboratory fee for any blood tests done in the office is the sole
responsibility of the patient. I
Also, if you happen to have medical insurance, it is very likely your insurance
will cover lab blood testing.
IMPORTANT NOTE:
The $100.00 initial fee this Does Not include cost of any vaccinations (if
vaccines are needed) or of any blood tests.
VACCINES:
Vaccines are very expensive. Adults need three. Children usually need more than
three. Dr. Sheta will inform you what vaccines will be required.

Insurance and Payment
Forms of payment:
We are happy to accept cash, MC/Visa cards, American Express
card or the Discover card. Payment in full is expected at the
time of your initial visit.

Payment in full is expected at the time of your initial visit.
However, you will receive an official medical receipt, which you can submit to
your insurance company for possible reimbursement. Please be advised that
private insurance companies in the USA usually do not pay for the 693
immigration medical exams.
Processing Time:
The USCIS 693 exam usually takes four to seven business days to
complete depending on your test results. Two day "RUSH" processing is available
in some cases for an additional RUSH fee.
