MEDICAL EXAM FOR CHANGE OF STATUS

What is the I-693 Exam?

Purpose of Form I-693:
Medical Examination of Aliens Seeking Adjustment of Status

Form I-693 must be filed by individuals that apply for adjustment of status. Form I-693 can only be completed by a physician that is approved by the United States Center for Immigration Services. All expenses related to the health check and completion of Form I-693 must be paid by the applicant. An adjustment of status application is not complete unless Form I-693 is included and approved by the physician. Dr. Sheta can help you complete your I-693 Exam.  Civil surgeons are responsible for ensuring that all of the required components of the I-693 form have been completed.  The civil surgeon will record the exam results on Form I-693 for the applicant to return to the USCIS (INS).

Each evaluation includes an examination performed according to the medical guidelines established by the USCIS and Centers for Disease Control; all required laboratory testing; TB testing and interpretation; and completion of all the necessary paperwork.  If necessary, vaccinations and chest x-rays are performed for an additional fee.

Dr. Sheta is experienced with the medical requirements of these examinations.  It is mandatory that immigration physicals are completed by a Designated Civil Surgeon so the exam will be valid.  To accommodate cultural preferences, male and female Designated Civil Surgeons are available.
