Welcome to our office, where our professional staff is readily available to meet your medical needs. The business office can assist you in meeting the financial obligations that go along with medical care. It is our office policy to receive payment in full at the time of service.

Insurance

Payment is requested for all office services at the time services are rendered, unless special arrangements are made with the office manager in advance. If you have insurance, we will file your claim with your insurance company. Please provide the receptionist with your current insurance card at the time of your appointment to avoid delay in filing claims.

Insurance -- We participate in many insurance plans, including Medicare. If you are not insured by a plan we do business with, payment in full is expected at each visit. If you are insured by a plan we do business with but don't have a current insurance card, payment in full for each visit is required until we can verify your coverage. Knowing your insurance benefits is your responsibility. Please contact your insurance company with any questions you have about your coverage.

Co-Payments and Deductibles -- All co-payments and deductibles must be paid at the time of service. This arrangement is part of your contract with your insurance company. Failure on our part to collect co-payments and deductibles from patients can be considered fraud. Please help us uphold the law by paying your co-payment at each visit.

Non-Covered Services -- Please be aware that some -- and perhaps all -- of the services you receive may be noncovered or not considered reasonable or necessary by Medicare or other insurers. You must pay for these services in full at the time of your appointment.

Proof of Insurance -- All patients must complete our patient information form before seeing the doctor. We must obtain a copy of your driver's license or picture ID, and current valid insurance card to provide proof of insurance. If you fail to provide us with the correct insurance information in a timely manner, you may be responsible for the balance of a claim.

Claims Submission -- We will submit your claims and assist you in any way we reasonably can to help get your claims paid. Your insurance company may need you to supply certain information directly. It is your responsibility to comply with their request. Please be aware that the balance of you claim is your responsibility regardless of whether your insurance company pays your claim. Your insurance benefit is a contract between you and your insurance company; we are not party to that contract.

Coverage Changes -- If your insurance changes, please notify us as soon as possible.

Statements and Nonpayment -- We strive to avoid mailing statements. Statements are mailed monthly to patients with balances due. Payment is due upon receipt.

If your account remains past due, we may refer you to a collection agency, and you and your immediate family members may be discharged from this practice. If this occurs, you will be notified by regular and certified mail that you have 30 days to find alternate medical care. During that 30-day period, our physician will only be able to treat you on an emergency basis.



Insurance and Billing

Bring your current insurance card(s) each time you visit our office. Many plans will not allow us to see you unless you present your card at the time of your appointment. We will accept your co-payment when you check in. Our office accepts cash and checks; and Visa, MasterCard, American Express and Discover credit cards, as well as debit cards displaying one of the credit card logos. Our service provider cannot accept ATM debit cards without logos..
